Zverev, who lifted the trophy in 2021, resumed with a 6-4, 5-4 lead and won the final four points to progress.
The third seed will face Toronto finalist Karen Khachanov later in the day in his bid for another deep run in the US Open warm-up event.
Women’s third seed Iga Swiatek booked her place in the quarter-finals with a 6-4, 6-3 win over Sorana Cirstea, overcoming 33 unforced errors in a match played in humid conditions.
Cincinnati remains one of just two WTA 1000 events where the former world number one has yet to reach a final.
Holger Rune also advanced after 2024 finalist Frances Tiafoe retired with lower back pain, trailing the Dane 6-4, 3-1.
Rune, who has battled his injury setbacks this season, secured his 100th career hard-court victory and his first win over a top-20 opponent since April.
Meanwhile, Poland’s Magda Linette pulled off a shock 7-6 (7-5), 3-6, 6-3 win over fourth seed Jessica Pegula to reach the fourth round for the first time in Cincinnati.
Linette, ranked 40th in the world, had never taken a set off Pegula before. The pair resumed their match tied at one set all, with Linette breaking for 5-3 before serving out the victory.
