A civil society organisation (CSO) that tracks government spending and projects, Tracka, has exposed the deplorable condition of Akarabata Primary Health Centre (PHC) in Modakeke town, Ife Central Local Government Area (LGA) of Osun state.
The CSO, after a recent visit to the Akarabata PHC, shared a statement on X, revealing that its team found the facility in a severely dilapidated condition.
According to Tracka, the PHC, which is often the first point of contact for many seeking healthcare, is marred by cracked walls and broken doors.
“We saw cracked walls, damaged roofing sheets, broken doors, missing window panes, and deteriorated flooring.
The facility manager explained that several reports have been submitted to the Primary Health Care Board highlighting the condition of the facility, but nothing has been done,” the statement read in part.

The facility has become vulnerable to vandalism and is reportedly occupied by hoodlums as a result of the poor state of the building. Residents of the community have no choice but to abandon the centre and travel to other facilities within the constituency to access healthcare services.
It also appealed to the Osun State Government, the National Primary Health Care Development Agency (NPHCDA) and other government agencies to renovate the PHC and make healthcare accessible for the residents of the community.
“We implore the @NphcdaNG, the@Osun_State_Gov, and the @Fmohnigeria to prioritise the urgent rehabilitation of this facility to restore access to primary healthcare for residents in the community,” the CSO added.
